Primary Care NJ is now recognized as a Patient-Centered Medical
Home (PCMH). A PCMH is a new approach to care. It is a
better way to care for you that starts right at your doctor's
office.
As a patient of a PCMH, you now have the added benefit
of being part of a medical practice that uses a team-based
approached, led by your doctor, for all your health care
needs. A PCMH practice leverages new health technology,
increasing your ability to stay in touch by phone and
e-mail to reach the health care staff about your plan
of care and more.
Stay connected! Using your PCMH as your first point of
contact allows you to receive a more coordinated and personalized
health care.
You can expect:
Quality, team-based care led by your doctor that's personalized,
friendly and responsive.
Coordinated care through your doctor, from start to finish.
Access to the care you need, when you need it.
Enhanced communications with your health care team by
phone, e-mail and in person.
Wellness and preventive care based on national guidelines.
Advanced technology to serve you better.
Extra support and resources to protect your health - including
health education.
These services will benefit those who receive routine
or preventive care but are especially important for patients
with chronic conditions or who have extra health care
needs. Remember to ask your doctor to suggest a specialist
when you need one. Your PCMH also has a Care Coordinator
to provide you with information and point you to additional
resources for taking care of your health. This means you'll
notice the difference when you need the help most.
Primary Care NJ is pleased to be a PCMH. You can look
forward to care that is more complete, better coordinated
and more personalized.
Thank you for choosing our practice for your medical care.
