Primary Care NJ is now recognized as a Patient-Centered Medical Home (PCMH). A PCMH is a new approach to care. It is a better way to care for you that starts right at your doctor's office.

As a patient of a PCMH, you now have the added benefit of being part of a medical practice that uses a team-based approached, led by your doctor, for all your health care needs. A PCMH practice leverages new health technology, increasing your ability to stay in touch by phone and e-mail to reach the health care staff about your plan of care and more.
